Job Description:
We are looking for a skilled and hard-working Physical Therapist to join our growing team.
The Physical Therapist will primarily be focused on planning and administering medically prescribed or referred physical therapy age specific treatment programs for patients to restore function, relieve pain, and prevent disability following disease, injury surgery or loss of body part.

Essential Functions:
1) Demonstrate and articulate sound clinical reasoning in synthesizing complex evaluation data, identifying diagnosis, and developing a comprehensive plan of care
2) Develop and update an appropriate plan of care in collaboration with the patient/family; related to their age and lifestyle with measurable long and short term goals
3) Utilize appropriate methods and materials for effective learning, monitoring the response to education, and adapting the program appropriately
4) Initiate patient and family education in a timely manner relative to age, readiness to learn, and emotional status
JOB REQUIREMENTS:
Must be able to pass a pre-employment drug screen and background check
State Issued License
Updated Resume
CPR Card (in person class only)
Physical from this year (This can be very basic, as simple as a quick prescription note from your doctor stating you are healthy and able to work)
TB from this year
TDAP (If you have one.)
Flu Vaccine (If it is flu season.)
MMR (Either 2 vaccinations or 1 positive Titer for Mumps, Rubella or Rubeola)
Hep B (If you have it)
Varicella (If you have it)
Two forms of ID (both ID's that you bring must have the same names. So if you were married it would have to show the same last name on both ID's.)
